Tooth necks do not consist of the strong tooth material of enamel, which is located at the crown of the tooth, but of dentin, which is much softer and has thousands of small tubules. These tubules lead deep into the dental pulp to the dental nerve. If a cold stimulus is felt on the tooth, these tubules transmit the stimulus to the dental nerve. This results in pain. On the other hand, other factors can also lead to the protective enamel being reduced, such as grinding.
